WNPC Puerto Rico Primary Care Wing
The Primary Care Wing at La Clinica de Doc Weston serves Mayaguez and the surrounding western Puerto Rico municipalities as a walk-in family practice -- filling a gap that, at most other WNPC sites, represents an underserved community's limited access to care. In western Puerto Rico, the gap is not limited access. It is disappearing access. The primary care infrastructure is collapsing in real time as physicians leave the island, and for many residents of the west coast, the WNPC primary care wing is not an additional resource. It is the only resource.
The primary care population carries the island's chronic disease burden at concentrated levels -- diabetes rates that exceed the mainland average, hypertension compounded by stress and poverty, chronic kidney disease, and the particular health legacy of Hurricane Maria: patients whose chronic conditions destabilized during the months without power, without medication refrigeration, without clean water, without medical care, and who never fully recovered the baseline they had before the storm. Post-Maria chronic health effects are a clinical category at the PR site that exists at no other WNPC location, because no other WNPC community has experienced a natural disaster of Maria's scale and duration of aftermath.
Medication access is a critical primary care function. The pharmacy infrastructure in western PR has contracted alongside the physician supply, and patients whose medications require refills face the same geographic barriers to pharmacy access that they face to physician access. The on-site pharmacy and the primary care wing's medication management program address this directly -- ensuring that a primary care patient leaves the wing with their medications in hand rather than with a prescription they cannot fill.
Everything operates in Spanish. The intake, the assessment, the explanation, the prescription instructions, the follow-up scheduling. A Mayaguez resident receives primary care in the language their body feels pain in.
